External Input -- VITA Web-based Consumer Evaluation

This evaluation questionnaire form provides a means for the Texas AMP VITA administrators to improve the services provided by this site, as a facilitative resource for all who are involved in Transfer and/or Articulation. It contains questions on the accessibility and usefulness of the material, the query process and other important factors regarding the search process. VITA requests that the consumer spend a few minutes completing the form, contributing to this site becoming more informative and user friendly.

It also contains a few questions, which should be rated "High", "Medium" or "Low" to indicate level of satisfaction. One may specify a choice in the same way by clicking the appropriate circle.
